Cardiology model v2.0

The cardiology model has been fully retrained on a broader dataset of cardiology-specific encounters. Notes generated for cardiology visits now reflect more accurate procedure documentation, tighter terminology for conditions like heart failure, arrhythmia, and valvular disease, and improved structure for Assessment and Plan sections that match how cardiologists actually write.
Physicians in cardiology practices reported that the previous model occasionally defaulted to general internal medicine phrasing. That is now corrected. The updated model recognises cardiology-specific shorthand, captures device and intervention history more reliably, and produces cleaner HCC-relevant diagnosis documentation out of the box.
No changes to how Diagnose runs in the background. The update is live for all cardiology users automatically.
Cerner integration

Diagnose now integrates directly with Cerner. Physicians at Cerner-based practices can connect their EHR during onboarding and have notes pushed directly into the chart without any copy-pasting or manual entry.
Our team handles the full setup. Most practices are live within 48 hours of signing. No IT involvement required on your end beyond an initial access approval.
This follows our Epic integration launched in Q4 2025 and brings our total supported EHR platforms to seven. If your practice runs on a system not yet listed, contact us and we will scope the integration.
Live Notes launched

Live Notes is a new panel that surfaces real-time clinical flags during an encounter. As the conversation unfolds, Diagnose identifies medication alerts, overdue tasks, and clinical conditions that may need attention and surfaces them quietly on screen without interrupting the visit.
The panel is designed to be glanceable, not distracting. Flags are categorised by urgency and dismissed with a single tap once addressed. Nothing is added to the note automatically. Live Notes is an in-visit reference tool only.
Live Notes is available to all active Diagnose users and is on by default. It can be turned off in settings if preferred.
Audio deletion confirmation log

Every Diagnose encounter now generates a per-encounter confirmation that audio was permanently deleted following note generation. The confirmation includes a timestamp, encounter ID, and deletion status, and is accessible from the encounter history screen.
This was built in response to physician and administrator feedback asking for visible proof that audio is not retained. The log does not change how Diagnose handles audio. Deletion on note completion has always been the default. It simply makes that process visible and auditable.
The confirmation log is available for all encounters from January 14, 2026 onwards. Historical encounters prior to this date do not have a retroactive log.
Psychiatry model v1.5

The psychiatry model has been updated with improved handling of sensitive clinical language, more accurate mental status examination documentation, and better structure for treatment plan sections specific to psychiatric encounters.
Previous versions occasionally over-documented patient disclosures in ways that required physician editing before signing. The updated model applies more clinical judgment to what belongs in the note versus what was contextual conversation. Psychiatrists using Diagnose reported a meaningful reduction in editing time following the update.
The update also improves accuracy for medication management visits, where brevity and precision matter more than length. Live for all psychiatry users automatically.
Diagnose is available on mobile

Diagnose mobile app is now available for iOS and Android. Physicians can review, edit, and sign notes directly from their phone between patients, in the hallway, or anywhere outside the clinic room.
The mobile experience is built around the same note review canvas as the desktop version. Sections collapse by default, Assessment and Plan open automatically, and the push to EHR action is available in one tap. Everything syncs in real time across devices.
